Frame Consent & Waiver

Some frames are just really special, and we love them so much that we want to wear them forever but unfortunately sometimes frames do not survive the reglazing process.

At Fitzroy North Eye Centre, we are happy to send your favourite frames off to our Lab partners to be reglazed with your current prescription and require you to agree to the following terms and conditions:

• I have requested that my existing frames be re-glazed with new prescription lenses
• I understand that although Fitzroy North Eye Centre will take all and proper care, the frame may break during the process required for re-glazing
• I agree that Fitzroy North Eye Centre have no financial or other responsibility should the frame break during re-glazing
• I accept the financial responsibility for any frame breakage
• I understand that if the frame breaks after one or both lenses have been cut to shape, then the lenses may be unusable in any other frame, and may have to be discarded
• I understand that should the frame survive re-glazing, it may not survive continued usage, in which case the lenses may have to be prematurely discarded 
• I agree to pay in advance for the lenses and a $50 reglazing fee.
• I acknowledge that I will not receive any refund or credit should the lenses have to be discarded
• Understand that if I don’t leave the frame with Fitzroy North Eye Centre, there is a risk the lens may not fit, and they will need my frame to send to the lab for final fitting, in which case I will be without my frame for 3 business days.
